Care finder program supports vulnerable older people who wouldn't be able to arrange services without intensive support, and don't have a family member, friend, or person they trust, to help.
Eligibility Criteria
Designed for vulnerable older people, this program might be for you if you: - Have no carer or support person who can help - Don't have a carer or support person you feel comfortable with or trust to support you - Are eligible for government-funded aged care Additionally, the care finder program might be for you if you: - Have difficulty communicating because of language or literacy problems - Find it difficult to understand information and make decisions - Are reluctant to engage with aged care or government - Are in a potentially unsafe situation if you don't receive these services
